All the recent schemes I've worked on have been TORR by track sequence:
the default has been berth and first tracks;
if that's not possible (e.g. no berth track or berth track is permissive) then second and third.
I do recall something about "unconditional" TORR but like PJW I think that is not for passenger lines. In the case where TORR is not possible as above (I can think of a track from a non-TC siding to a permissive platform with only 3 tracks) NR have generally accepted that TORR will not be provided.
